By signing this agreement, I confirm that I am voluntarily surrendering the dog described in this agreement to Snow Dog Rescue Society.
I acknowledge and agree to the following:
I am the legal owner of the dog or have the legal authority and permission to surrender the dog to Snow Dog Rescue Society.
I voluntarily and permanently relinquish all ownership rights, title, interest, and claims to this dog. Ownership transfers to Snow Dog Rescue Society upon acceptance of the dog.
I understand that this surrender is permanent and irrevocable. Once the dog has been accepted into the care of Snow Dog Rescue Society, I cannot reclaim the dog or request that it be returned.
I understand that Snow Dog Rescue Society will make all decisions regarding the dog's care, veterinary treatment, transportation, foster placement, behavioural rehabilitation, adoption, transfer to another reputable rescue, or, if deemed medically or behaviourally necessary by a licensed veterinarian or the rescue, humane euthanasia.
I understand that Snow Dog Rescue Society cannot guarantee that every surrendered dog will be placed for adoption. Each dog will be assessed based on its medical condition, behaviour, and quality of life.
I agree to provide complete and honest information regarding the dog's health, age, temperament, bite history, behaviour, previous ownership, and any known medical or behavioural concerns. I understand that withholding or providing false information may place people or other animals at risk and may increase costs for the rescue.
I authorize Snow Dog Rescue Society to obtain veterinary care, vaccinations, parasite treatment, sterilization, diagnostic testing, surgery, emergency treatment, or any other medical care deemed necessary.
I understand that Snow Dog Rescue Society may rename the dog, update its records, microchip the dog, and represent the dog publicly through photographs, videos, and social media for fundraising, educational, or adoption purposes.
I acknowledge that northern community dogs often require transportation by air, road, or ferry and may spend time in temporary foster homes or boarding facilities before finding permanent homes.
To the best of my knowledge, I have disclosed any known history of aggression, bites, attacks, contagious illness, or dangerous behaviour.
I release and forever discharge Snow Dog Rescue Society, its directors, volunteers, foster homes, veterinarians, transport volunteers, and partner organizations from any liability, claims, damages, losses, or expenses arising from the care, transport, treatment, behaviour, adoption, or disposition of the surrendered dog after ownership has been transferred.
I understand that acceptance of a dog is at the sole discretion of Snow Dog Rescue Society. Submission of this agreement does not guarantee that the rescue has immediate capacity to accept the dog.
I certify that all information I have provided is true and accurate to the best of my knowledge.
By signing, I acknowledge that I have read, understood, and voluntarily agree to these Terms and Conditions, and I permanently surrender ownership of this dog to Snow Dog Rescue Society.
