By signing this agreement, I confirm that I am voluntarily returning a dog previously adopted through Snow Dog Rescue Society.
I acknowledge and agree to the following:
I am the current legal owner of the dog and have the authority to surrender the dog back to Snow Dog Rescue Society.
I understand that my Adoption Agreement requires that, if I am no longer able or willing to care for the dog, the dog must be returned to Snow Dog Rescue Society and may not be rehomed, sold, given away, surrendered to another rescue or shelter, or transferred to any other person without the prior written consent of Snow Dog Rescue Society.
By signing this agreement, I voluntarily and permanently relinquish all ownership rights, title, interest, and claims to the dog. Ownership transfers back to Snow Dog Rescue Society upon acceptance of the dog.
I understand that this surrender is permanent and irrevocable. Once the dog has been accepted back into the care of Snow Dog Rescue Society, I have no further ownership rights and cannot request the dog's return.
I understand that Snow Dog Rescue Society will determine the most appropriate course of action for the dog, including veterinary care, behavioural assessment, foster placement, rehabilitation, transfer to another reputable rescue partner, or adoption into a new permanent home.
I understand that while every reasonable effort will be made to rehabilitate and rehome the dog, Snow Dog Rescue Society cannot guarantee that every returned dog is suitable for adoption. Decisions regarding the dog's future will be made based on veterinary recommendations, behavioural assessments, safety considerations, and the dog's overall quality of life.
I agree to provide complete, honest, and accurate information regarding the reason for surrender, the dog's medical history, behavioural history, training, temperament, bite history, interactions with people and animals, and any other information that may assist the rescue in providing appropriate care and placement.
I understand that withholding or providing false or misleading information may place volunteers, foster homes, adopters, other animals, or the dog itself at risk.
I authorize Snow Dog Rescue Society to obtain any veterinary care deemed necessary, including vaccinations, parasite treatment, diagnostic testing, surgery, sterilization (if not already completed), emergency treatment, behavioural evaluation, medications, or humane euthanasia if recommended by a licensed veterinarian or deemed necessary for the welfare of the dog or public safety.
I understand that the dog may be transported, boarded, placed in one or more foster homes, or relocated within or outside my province as part of the rescue's placement process.
I acknowledge that Snow Dog Rescue Society may photograph, video, rename, microchip, and publicly share information about the dog for adoption, fundraising, educational, promotional, or rescue purposes.
I release and forever discharge Snow Dog Rescue Society, its directors, officers, volunteers, foster homes, veterinarians, transport volunteers, partner organizations, and representatives from any liability, claims, damages, losses, or expenses arising after ownership has been transferred back to the rescue.
I understand that any adoption donation originally paid is non-refundable unless otherwise stated in the Adoption Agreement or approved in writing by Snow Dog Rescue Society.
I certify that all information I have provided is complete and truthful to the best of my knowledge.
By signing below, I acknowledge that I have read, understood, and voluntarily agree to these Terms and Conditions. I permanently surrender ownership of this dog back to Snow Dog Rescue Society and authorize the rescue to make all decisions regarding the dog's future care and placement in its sole discretion.
